We ask your understanding and cooperation with these policies:
All patients should bring their insurance cards and photo identification to each appointment. We require that each patient provide this information each time there are seen.
We would like to remind patients who are more than 10 minutes late for their appointment that we may need to reschedule your visit.
All new patients will need to fill out forms prior to being seen by the midwife. These forms can be downloaded prior to coming to your appointment. Please plan on arriving 15 minutes ahead of your scheduled appointment with your forms, insurance card and photo identification.
If it has been more than a year since your last visit, or if you’re being seen for an annual wellness exam, you will need to fill out updated forms. Please bring these forms, your insurance card, and photo identification to your appointment.
Lab Work Policy:
We do offer the convenience of phlebotomy services in the office for our patients. Lab results will be reported to patients by phone only if they have abnormal results which require some intervention, such as a prescription or a re-test. Normal results will be reported at the next routine office visit and will be uploaded into your patient portal along with a note from your midwife.
Not all insurances are the same. Each company offers many different kinds of policies with different types of benefits. It is important that you call your insurance company prior to coming in for your visit. Things you should ask:
- Are these providers in-network with my plan?
- Will I owe a copay?
- Do I have a deductible and/or a co-insurance?
- Is a referral necessary?
It is a patient’s responsibility to know her plan and be prepared for any out-of-pocket expenses that may be owed for care provided.
Patients who “no-show” for their scheduled visit will be charged $50.00.
Copays & Balances
Copays are due at the time of service. Copays are indicated on most insurance cards. If you are unsure if you owe a copay, please contact your insurance company for verification. Balances owed to Loudoun Community Midwives are paid at your office visit. If you are unable to pay at the time of your visit, please talk to our front office staff about setting up a monthly payment plan.
Loudoun Community Midwives is part of the Loudoun Medical Group located in Leesburg, Virginia. LMG handles all of our billing and many other business issues. Currently, we participate with most insurance carriers. If you would like to confirm that we accept your insurance, please call our office and inquire with the front desk. We are NOT providers for Kaiser Permanente. Any co-pays required by patient are due at time of appointment. If you have questions, please contact us.
